To apply for a Savings Program payout, you should call Participant Services at 1-800-777-4015. If you die with a remaining balance in the Plan, your beneficiaries should contact The Recordkeeper for information on obtaining a distribution.

If you elect a lump sum payout, you will be mailed the payout generally within three business days from the date Participant Services receives the request. If you elect to receive installment payments, you will receive the required forms to complete and return.
